This notice is being provided to inform you that Bacharach Institute for Rehabilitation inpatient rehabilitation hospital, Renaissance Pavilion Long Term Care facility and Bacharach Rehab Medicine and all outpatient services were closed effective March 31, 2023.

Medical records are stored with Desert River Solutions. After January 2, 2023 all patient requests will be handled by Desert River Solutions. To request your medical record please visit the site below to fill out and sign a medical release form. All instructions are on that webpage:
